Authorization for Treatment
I authorize the practitioners of Serene Skin Club to perform the following treatments: Pharmaceutical Grade Facial Peels, Facials, Dermaplaning, Microneedling, Microdermabrasion, Injectables, Radio Frequency, and/or 5G Hyperpigmentation Treatment. These treatments aim to improve or treat the following conditions: exfoliate or remove outer layers of skin, hyperpigmentation, rosacea, skin tightening, body contouring, and skin rejuvenation.
Understanding of Risks and Benefits
I understand that there is a rare possibility of side effects or serious complications, including permanent discoloration and scarring. I acknowledge that careful adherence to all advised instructions will help reduce this possibility.

- No Guarantee: No precise results or discomfort levels are guaranteed.
Consent to Treatment
I understand that results may vary, and it is impossible to predict individual responses or the number of sessions required. I consent to photographs for documenting progress, kept solely in my medical record.
Acknowledgment and Release of Liability
I acknowledge and accept that the treatment involves known and unknown risks, including but not limited to discomfort, allergic reactions, and unexpected outcomes. I release, waive, and discharge Serene Skin Club, its owners, employees, and affiliates from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of Serene Skin Club or otherwise.
Assumption of Risks
I understand and voluntarily assume all risks related to the treatments provided by Serene Skin Club, including risks not specifically enumerated in this form, to the fullest extent permitted by law.
Indemnification I agree to indemnify and hold harmless Serene Skin Club, its owners, employees, and affiliates from and against any and all claims, liabilities, damages, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising from or related to my participation in the treatments.
